NAPA Contractees

This page is specific to NAPA’s county contractees, listed above, and contains explicit documents for their worksite wellness sites.

Below is a sequential list of documents in the exact order for implementation into your worksites.  Meaning, each of these six documents is strongly suggested for your use in the chosen sites of your 2009-2010 workplans.

Required documents:

4. Employee Focus Group/Survey Questions (word doc 27kb)

6. Exit Employer Discussion Guide (word doc 26KB)

Summary of documents:

  1. The Employer Worksite Questionnaire will be used for your initial discussion with the employer.  It contains questions to ask the employer pertaining to developing their worksite wellness program.
  2. MOU is a “contract” between your county health department and the worksite wellness site.
  3. This strategy checklist will be used to assess which physical activity and nutrition strategies are already in place at the worksite wellness site.
  4. This guide will be used during your focus group sessions or surveys with the employees.
  5. This strategy checklist is the same as number 3.  Now to be used to see how many more strategies were implemented for the wellness program.
  6. This discussion guide provides you with questions to ask the employer after the wellness program has been implemented.  It contains ‘prompts’ to help you facilitate the discussion.
